Parental Permission to Participate in NARRA Online Tutoring Program

I give my child, permission to attend the NARRA Online Tutoring Program.

I will make sure that there will be a presence of an adult during the tutoring session of my child

I grant NARRA all right, title and interest in any and all photographs, images, video and audio recordings of my child or his/her likeness or voice made by NARRA in connections with the NARRA Tutoring Program and to publish and/or display photographs of my child to promote the NARRA Tutoring Program.

I understand that the tutoring sessions are being offered through the ZOOM or similar online meeting platform and each session will be recorded and supervised by Program Director.

I understand that the tutoring program is on a first come first serve basis. I also understand that NARRA will give confirmation of my childs placement in the program.
